As the recent era of human civilization is in direct relation of the carrier of human medicine, actually MD graduates have to follow specialty and subspecialty programs. Even general practitioners nowadays need to get specialty in family medicine to start independent medical practice. 
So starting MD carrier will be initially through residency programs in vast range of specialties to get then certified to practice their carrier. 
In Palestine family medicine specialty should be encouraged and emphasized as it is so much deficient in our community in spite of its crucial role and vital position in health care infrastructure and development. 
Specialization programs need in general 4-6 years with some programs needing more.
